_______________________, Requestor, hereby enters into this Contract for Process Service with P.I. Solutions Group, hereinafter referred to as PISG, and freely, willingly, and without recourse, accepts the following terms:

ÿ Service of Process Rural Areas or areas not listed: Call for pricing.
ÿ High Risk Serves $100 - $200 (i.e., Temp Restraining Orders/Dissolutions/Problem Tenant)
ÿ Rush Handling $25 Extra (1st Attempt within 24 hours) Call for availability.
ÿ Stake Out/Surveillance for Service $75/hr (2 hr minimum)
ÿ Documents Picked Up for Service and/or Posting $25
ÿ Notarized Proof of Service $30 extra first signature; $20 two or more signatures (if requested)
ÿ Printing Charges 20¢ per page over 25 pages
ÿ Other: $
All costs and fees paid to PISG for process service are non-refundable once received except through specific written Agreement signed by both parties and shall be paid in advance.
I agree to the terms and conditions outlined above and release PISG from any claim or liability or in the event personal service cannot be affected.
